Homelessness and Addiction Recovery Treatment (HART) Hubs are a provincial initiative to support people with complex mental health, substance use, and housing needs in Ontario. HART Hubs are collaborative initiatives, requiring co-location and integration of health and social services. Each Hub is unique and designed to provide the services required to meet its specific community’s needs.
The Guelph-Wellington HART Hub is a collaborative project involving multiple health and housing partners. The Hub will include an Intensive Housing and Treatment Team and Integrated Crisis Centre, co-located at Guelph Community Health Centre in downtown Guelph. Services will be offered throughout the County of Wellington through a satellite location and outreach. The Intensive Housing and Treatment Team will provide wraparound physical, mental, and substance use healthcare—as well as connections to social support such as housing and income. The Integrated Crisis Centre will include a 24/7 integrated crisis service, crisis stabilization beds, and medically supported withdrawal management beds.
In October 2024, Toward Common Ground—in partnership with Guelph Community Health Centre—coordinated and wrote the application for the Guelph-Wellington HART Hub. In January 2025, it was announced that the application was successful and over $18 million in provincial funding would be awarded over three years to implement the project.
The Guelph-Wellington HART Hub is scheduled to open in April 2025.
